Where and how can I apply for a CGHS Card (for pensioners)?

Online Process:

1. Visit the official CGHS portal: https://cghs.nic.in

2. Click on the “Apply for CGHS Card” link under the Beneficiaries → Pensioners section

3. Fill out the application and upload:

   • Pension Payment Order (PPO)

   • Aadhaar Card

   • Passport-size photograph and signature

   • Residential proof

4. Take a printout and visit the nearest Additional Director, CGHS Office for verification and biometric data submission.

Offline Option:

You can collect and submit the form directly at your nearest CGHS Wellness Centre or the Additional Director’s Office with all necessary documents.

How Much Do You Need to Pay?

You have two options based on your last drawn basic pay:

1. Annual Contribution:

   • ₹6,500–₹8,499 → ₹600/month or ₹7,200/year

   • ₹8,500–₹11,499 → ₹850/month or ₹10,200/year

2. One-Time Lifetime Contribution:

   • ₹72,000 for ₹6,500–₹8,499 slab

   • ₹1,02,000 for ₹8,500–₹11,499 slab
