Thank you for your question. As a 64-year-old pensioner of the Employees' Provident Fund Organisation (EPFO), you are eligible to apply for a CGHS card to avail of cashless treatment in CGHS-empanelled hospitals.
Visit the official CGHS portal: https://cghs.nic.in
Click on the “Apply for CGHS Card” link under the Beneficiaries → Pensioners section
Fill out the application and upload:
Pension Payment Order (PPO)
Aadhaar Card
Passport-size photograph and signature
Residential proof
Take a printout and visit the nearest Additional Director, CGHS Office for verification and biometric data submission.
You can collect and submit the form directly at your nearest CGHS Wellness Centre or the Additional Director’s Office with all necessary documents.
You have two options based on your last drawn basic pay:
Annual Contribution:
₹6,500–₹8,499 → ₹600/month or ₹7,200/year
₹8,500–₹11,499 → ₹850/month or ₹10,200/year
One-Time Lifetime Contribution:
₹72,000 for ₹6,500–₹8,499 slab
₹1,02,000 for ₹8,500–₹11,499 slab
