How to Apply for an Ayushman Bharat (PM-JAY) Health Card?

If you want to apply for an Ayushman Bharat (PM-JAY) Health Card, the first and most important step is to check whether you or your family are eligible for the scheme. The Ayushman Card is issued only to eligible beneficiaries identified under the PM-JAY database and other approved categories. 

Step 1: Check Your Eligibility

You can check your eligibility through the official PM-JAY beneficiary portal

Official Portal :

Ayushman Bharat PM-JAY Beneficiary Portal

Follow these steps :

1. Visit the beneficiary portal.

2. Select "Beneficiary" login.

3. Enter your mobile number and verify it using the OTP.

4. Select your state.

5. Search using any of the following :

   • Aadhaar number

   • Ration card number

   • Family ID

   • Name and district

6. If your name appears, you are eligible to proceed with the Ayushman Card application.

Step 2: Keep the Required Documents Ready

You may need the following documents :

• Aadhaar Card

• Ration Card (if applicable)

• Family ID/Household documents (if available)

• Mobile number linked with Aadhaar

• Any other identity document requested during verification

Step 3: Complete e-KYC Verification

Once your eligibility is confirmed :

1. Click on the e-KYC option next to your name.

2. Verify your identity using Aadhaar OTP or biometric authentication.

3. Upload or verify your photograph and other details.

4. Submit the application for approval.

Step 4: Download Your Ayushman Card

After successful verification and approval :

1. Log in again to the beneficiary portal.

2. Select the approved beneficiary.

3. Click "Download Card".

4. Save or print your Ayushman Card for future use.

If You Cannot Apply Online

You can also visit :

• Your nearest Common Service Centre (CSC)

• Any PM-JAY empanelled government or private hospital

• The Ayushman Mitra help desk at the hospital

They can help you check eligibility and generate the Ayushman Card. 

Important Points to Remember

• The Ayushman Card is issued free of cost.

• Do not pay money to any agent for card generation.

• The card provides cashless treatment coverage up to ₹5 lakh per eligible family per year at PM-JAY empanelled hospitals.
