Deep, sharp or pointed pain inside one ear that becomes worse during the day, along with sensitivity to loud sounds, can have several possible causes. Is MVD Surgery a Good Option?

Microvascular decompression (MVD) is a major neurosurgical procedure in which a blood vessel compressing a cranial nerve is separated from the nerve. It is most commonly considered for specific nerve-compression conditions, particularly classical trigeminal neuralgia. Guidelines recommend MRI as part of the evaluation and state that evidence of neurovascular contact can help determine whether MVD is appropriate.

Therefore, MVD should not be considered simply because the ear pain has continued despite medicines.

Before considering MVD, a specialist should establish

• Which nerve is responsible for the pain.

• Whether your symptoms actually fit a neuralgia pattern.

• Whether there is evidence of neurovascular compression.

• Whether the compression is on the same side as your symptoms.

• Whether other causes of ear pain have been ruled out.

• Whether appropriate medical treatments have genuinely failed or were not tolerated.

• Whether you are medically fit for major neurosurgery.

High-resolution MRI/MRA focused on the relevant cranial nerves may be required. Importantly, MRI findings alone do not prove that a nerve is causing the pain; they need to be interpreted alongside your symptoms and clinical examination.

What Should You Do Next?

Because your symptoms have persisted despite treatment from ENT and neurology specialists, consider getting a consultation from a neurologist who specialises in cranial neuralgias or a neurosurgeon experienced in cranial nerve disorders.

Take all your previous records to the consultation, including

• Previous ENT examination reports

• Neurology consultation records

• MRI/CT scans and the actual images, not only the reports

• Hearing or audiometry reports

• Previous medication prescriptions

• Details of which medicines helped or failed

• Any records mentioning nerve compression or vascular contact

The specialist recommend additional high-resolution imaging or other tests before deciding on treatment.

Important Regarding Your Sound Sensitivity

Sensitivity to loud sounds should also be evaluated separately. An ENT or audiologist can assess your hearing and middle/inner-ear function to determine whether there is an underlying auditory problem. It is important not to assume that sound sensitivity is caused by a nerve compression without appropriate evaluation.

When Should You Seek Urgent Care?

Seek prompt medical attention if the pain is accompanied by sudden hearing loss, severe dizziness, facial weakness, new neurological symptoms, severe headache, fever, ear discharge, or rapidly worsening symptoms.
