How to download and retrieve a lost Ayushman Bharat card?

If your Ayushman Bharat (PM-JAY) Card is lost, there's no need to worry. You can easily download, reprint, or get a duplicate copy of your card. As long as you are an eligible PM-JAY beneficiary, your details remain available in the system, and you can access your card again.

Option 1: Download Your Ayushman Card Online

You can download your Ayushman Card by following these steps

• Visit the official Ayushman Bharat PM-JAY Beneficiary Portal.

• Log in using your registered mobile number and verify it with the OTP.

• Select your State and search using your Aadhaar Number or Family ID.

• Once your details appear, verify your identity through Aadhaar OTP.

• Download and save your Ayushman Card or print it for future use.

Option 2: Download Using the Ayushman App

You can also use the official Ayushman App to access your card.

• Log in with your registered mobile number.

• Verify your identity using OTP.

• Search your beneficiary details using your Aadhaar or Family ID.

• Complete the verification process and download your digital Ayushman Card.

Option 3: Get a Duplicate Card Offline

If you're unable to download the card online, you can visit

• A nearby Common Service Centre (CSC).

• The Ayushman Bharat Help Desk (Ayushman Mitra) at any PM-JAY empanelled hospital.

Carry your Aadhaar Card or another valid identity document. The staff will verify your details and help you print a duplicate Ayushman Card.
