Ayushman Card Not Found in The Record, how do I get it?

Ayushman Card Not Found does not always mean you are ineligible for the scheme. It may happen because your details are not yet linked to the PM-JAY database, there is a mismatch in your records, or your beneficiary information has not been updated.
The first step is to verify your details and find out why your record is not appearing before applying for any correction.
Some common reasons include
Your family is not yet included in the PM-JAY beneficiary database.
Your Aadhaar or mobile number is not linked correctly.
There is a mismatch in your name, age, or other personal details.
Your Ration Card or Family ID information has not been updated.
You are searching using incorrect details.
Before assuming your card is unavailable, try searching using different details such as
Aadhaar Number
Family ID
Ration Card Number
Registered Mobile Number
State, District, and Village/City details
Sometimes your record appears when searched using an alternative document.
If your details still cannot be found online, visit your nearest Common Service Centre (CSC).

Aadhaar Card
Ration Card (if available)
Registered Mobile Number
Any other government-issued identity proof
