Steps to Check a PM-JAY Empanelled Hospital

1. Open the official PM-JAY Hospital Search portal.
2. Select your State.
3. Select your District.
4. Select the Hospital Type Public or Private.
5. Select the relevant Speciality, such as Ophthalmology.
6. Enter the Hospital Name, if you already know it.
7. Enter the Captcha shown on the screen.
8. Click Search and check whether the hospital appears in the results.

Documents to carry

• Ayushman Card
• Aadhaar Card or valid ID proof
• Doctor's prescription
• Eye examination reports, if available
• Previous medical records, if any
