How to Check Hospital Under Ayushman Card



Before travelling for surgery, follow these steps


  1. Open the official PM-JAY hospital search portal.

  2. Select Tamil Nadu as the state.

  3. Select Vellore as the district.

  4. Search for Christian Medical College and Hospital, Vellore.

  5. Check that the hospital's current PM-JAY empanelment is active.

  6. Check the General Surgery speciality.


Documents Required


Carry these documents when visiting the hospital


  • Ayushman Bharat PM-JAY Card, if you have one

  • ABHA Card/ABHA number, if available

  • Aadhaar Card or valid government ID

  • Applicable Tamil Nadu health-scheme card, if enrolled

  • Cardiologist's prescription

  • Coronary angiography/CAG report and CD

  • ECG report

  • 2D Echo report

  • Blood-test reports

  • Previous discharge summaries

  • Previous angioplasty/stent records, if any

  • Current medicines and prescriptions


If the cardiologist has specifically advised a stent, carry the written recommendation and angiography report.


If the Patient Has Chest Pain


If the patient currently has severe or persistent chest pain, breathlessness, sweating, fainting, or pain spreading to the arm, jaw, shoulder or back, please seek emergency cardiac care immediately. Do not delay urgent treatment while arranging insurance documents.
