Agar aap abdominal hysterectomy (bachhedani nikalne ka operation) ke liye documents pooch rahe hain, to documents is baat par depend karte hain ki aap Ayushman Card se treatment karwa rahe hain ya normally private hospital mein treatment le rahe hain.

Agar Ayushman Card Se Operation Karwana Hai

Hospital ke Ayushman Mitra/PM-JAY desk par generally ye documents maange ja sakte hain :

• Ayushman Card

• Aadhaar Card ya valid ID proof

• Gynaecologist ki prescription aur operation ki advice

• Ultrasound report

• Blood test aur other investigation reports

• Previous medical records aur prescriptions

• Previous discharge summary, agar available ho

• Referral/admission papers, agar required ho

Hospital in documents ke basis par eligibility aur applicable hysterectomy package check karega. Zarurat padne par pre-authorisation bhi process kiya ja sakta hai.
