How to Check an Ayushman Hospital

1. Visit the official PM-JAY Hospital Search.

2. Select your State.

3. Select your District.

4. Select Hospital Type like public , private.

5. Select the relevant speciality as Nephrology.

6. If you know a specific hospital, enter its name.

7. Fill in the Captcha.

8. Click on Search.

9. The list of PM-JAY empanelled hospitals will appear.

Documents Required

• Ayushman Bharat PM-JAY Card/e-card

• Aadhaar Card or valid ID proof

• Doctor’s prescription/referral

• Kidney function test reports

• Blood and urine test reports

• Ultrasound/CT scan reports, if available

• Previous medical records and discharge summaries

• Dialysis records, if applicable
