How to check an Ayushman hospital for dialysis

Follow these steps :

1. Go to the official Ayushman Bharat PM-JAY hospital search facility.

2. Select your State.

3. Select your District.

4. Select the hospital type, such as Government or Private.

5. Look for the relevant specialty/service related to dialysis or nephrology.

6. Search for the hospital by its exact name if you any in mind.

7. Enter Captcha

8. Click on search

9. Check whether the hospital is currently shown as PM-JAY empanelled.

10. You have to check overall hospital also of your district under pmjay

    
    

Documents to carry

Keep these documents with you :

• Ayushman Card

• Aadhaar Card or other valid identity proof

• Doctor’s prescription/advice for dialysis

• Kidney disease/CKD reports

• Previous dialysis records, if available

• Blood test reports

• Ultrasound/CT or other relevant reports

• Previous hospital discharge papers, if applicable

• Mobile phone linked with your beneficiary details, if required for verification

The hospital will verify your beneficiary details before providing treatment under PM-JAY.
