How to Find an Ayushman Hospital for Stroke Treatment

1. Open the official PM-JAY Hospital Search portal.
2. Select your State.
3. Select your District/City.
4. Select Government or Private Hospital, as applicable.
5. Select Neurology/Neurosurgery or the relevant speciality.
6. Enter the hospital name if you already have one in mind.
7. Enter the Captcha.
8. Click Search.
9. Check the list of hospital is currently PM-JAY empanelled.

Documents to Carry

• Ayushman Bharat PM-JAY Card/e-card
• Aadhaar Card or valid government ID
• Ration Card/Family ID, if available
• Doctor's prescription or referral
• CT Scan/MRI brain reports and images, if available
• Blood test reports
• Previous medical records
• Previous prescriptions and discharge summaries
