How to Apply for a New Ayushman Card When the Application Is Not Being Approved?

If you are trying to apply for a new Ayushman Bharat Card but are unable to get it, do not worry. First, check whether your name is included in the Ayushman Bharat beneficiary list. If your name is listed, you can complete the required verification and apply for your card.

How to apply for a new Ayushman Card online?

1. Open the official Ayushman Bharat Beneficiary Portal.
2. Select the Beneficiary option.
3. Enter your mobile number and log in using the OTP sent to your phone.
4. Select your state and the relevant scheme.
5. Search for your name using the available details, such as your Aadhaar number or family ID, if supported.
6. If your name appears in the list, select it and follow the instructions to complete your e-KYC verification.
7. Submit the application and check its status on the portal.
8. Once your card is approved and generated, download it from the portal.

What should you do if your Ayushman Card is not being generated?

• Your name is not showing: Check your details carefully and try another available search option. If your name is still missing, contact an Ayushman Mitra at an empanelled hospital or visit an authorised Common Service Centre (CSC) to check your eligibility.
• You are not receiving the OTP: Check whether your mobile number is active. If Aadhaar verification is required, ask an authorised centre about updating your mobile number or using another available verification method.
• Your application is pending or rejected: Check the status and the reason shown on the portal. Contact an Ayushman Mitra or the scheme helpline for help with the next steps.
• Your family member already has an Ayushman Card: Ask the help desk to check whether your name is included in the eligible family record.

What documents required?

• Aadhaar card or another accepted identity document.
• Ration card or family ID, if required under your state's scheme.
• Mobile phone for OTP verification, if applicable.
• Other documents requested by the authorised centre to verify your eligibility.

Important: Having an Aadhaar card or ration card alone does not guarantee eligibility for PM-JAY. Your eligibility must be confirmed through the official system. People aged 70 years and above may be eligible under the separate Ayushman Vay Vandana Card scheme, subject to its rules.
