How to Check a Hospital for Knee Replacement

1. Open the official PM-JAY Hospital Search portal.
2. Select your State.
3. Select your District.
4. Choose Government or Private Hospital.
5. Select the Orthopaedics specialty.
6. Enter the hospital name if you already have a hospital in mind.
7. Complete the Captcha.
8. Click Search.
9. Check whether the hospital has active PM-JAY empanelment and Orthopaedics services.
10. Check proper list of empanelled hospitals of your district

Documents Required

Your father should keep these documents ready:

• Ayushman Bharat PM-JAY Card
• Aadhaar Card or accepted government ID
• Orthopaedic doctor's prescription/referral
• X-rays of both knees
• MRI/CT reports, if advised
• Blood test reports, if available
• Previous medical records
• Previous hospital discharge summaries, if any
• Current medicines and prescriptions
• Any other documents requested by the hospital's Ayushman Mitra
