Yes. Health City Hospital, Guwahati is an Ayushman Bharat/PM-JAY empanelled hospital.

Documents to carry

• Ayushman Card/e-card

• Aadhaar Card or valid government ID

• Doctor's prescription/admission advice

• Ultrasound/CT scan report, if done

• Blood test reports

• Previous medical records

• Previous discharge papers, if any

• Current medicines/prescriptions

Since you are from Shillong, Meghalaya, you do not need to travel back to Meghalaya just because your Ayushman Card is from there, provided the patient is eligible and the Guwahati hospital accepts the applicable PM-JAY package under portability.
