Before taking the injection, follow these steps

1. Open the official PM-JAY hospital search portal.

2. Select Karnataka.

3. Select Bengaluru Urban.

4. Select/search Ophthalmology.

5. Search the exact hospital name.

6. Confirm that PM-JAY empanelment is active.

Documents Required

Carry:

• Ayushman Bharat PM-JAY Card/e-card

• Aadhaar Card or valid ID

• Ophthalmologist's prescription

• Retinal examination report

• OCT scan/report

• Fundus examination report
