How to Check the Hospital

Before taking your mother for surgery

1. Open the official PM-JAY empanelled-hospital search facility.

2. Select your state and district.

3. Search the hospital by its exact name.

4. Check that its PM-JAY empanelment is currently active.

5. Check that Ophthalmology services are available.

• Ayushman Bharat PM-JAY Card/e-card

• Aadhaar Card or valid identity proof

• Eye doctor's prescription

• Cataract/eye examination reports

• Previous eye-treatment records

• Blood-test reports, if available

• Other medical reports advised by the doctor

• Previous discharge summary, if applicable

• Current medicines/prescriptions
