What documents are needed for eye treatment in elderly patients?

A 69-year-old woman can carry some basic documents for a normal eye consultation, and if she wants to use an Ayushman card, a few additional documents are helpful.

Documents for Normal Eye Treatment

For a routine eye check-up, take :

• Aadhaar card or another valid ID

• Previous eye prescription, if available

• Previous eye test reports

• Previous glasses prescription

• Details of current medicines

• Previous hospital or treatment records, if any

• Mobile phone for appointment or registration, if required

If she already has cataract, glaucoma, diabetes-related eye problems or any other diagnosed eye condition, carry all previous eye reports and scans. Eye hospitals may also have their own registration or appointment requirements.

Documents for Ayushman Card Treatment

If she wants to take eligible eye treatment under Ayushman Bharat PM-JAY, keep :

• Ayushman card, if already issued

• Aadhaar card

• Ration card or family ID, if available

• Registered mobile phone

• Doctor's prescription/referral

• Previous eye reports and scans

• Previous hospital discharge papers, if any

• Current medicines list

Under PM-JAY beneficiary verification, Aadhaar or another permitted government ID and family identification documents such as a ration card may be used, depending on the applicable verification process.

If She Does Not Have an Ayushman Card

At 69 years of age, she does not qualify for the separate PM-JAY senior-citizen benefit that is specifically for people aged 70 years and above. However, she still be eligible under the regular PM-JAY/state eligibility rules. The hospital's Ayushman Mitra can check her eligibility.

For Ayushman treatment, make sure the hospital is actively empanelled and confirm that the specific eye treatment or procedure is covered under an applicable PM-JAY package. Routine outpatient care is generally not covered under PM-JAY.
