2. Select the patient’s State.

3. Select the District.

4. Select Government or Private Hospital.

5. Select the relevant eye/ophthalmology specialty.

6. You can also check the list of Ayushman-empanelled hospitals available in your district.

7. If you have a particular hospital in mind, enter the exact hospital name.

8. Complete the captcha, if shown, and click Search.

9. Check that the hospital has active PM-JAY empanelment.

10. Visit the hospital’s Ayushman Mitra/PM-JAY desk and confirm whether the specific glaucoma treatment or operation advised by the eye doctor is covered and whether pre-authorisation is required.
