Ayushman hospital check karne ke proper steps

1. Official PM-JAY Hospital Search portal open karein.

2. Apna State select karein.

3. Apna District select karein.

4. Government/Private Hospital select karein.

5. Specialty mein Ophthalmology/Eye Care select karein.

6. Hospital ka naam enter karein.

7. Captcha, agar shown ho, complete karein.

8. Search par click karein.

9. Check karein ki hospital ka PM-JAY empanelment active hai.

10. Hospital ke Ayushman Mitra se confirm karein ki cataract operation ka specific package available hai.

Aap overall empanelled hospital list bhi check kar sakte hain, jisse apne area mein available Ayushman hospitals dekh saken.

Documents lekar jaayein

• Ayushman Card

• Aadhaar Card/valid ID

• Eye specialist ki prescription

• Eye examination reports

• Previous medical reports, agar available hain

• Previous discharge papers, agar applicable hain
