Steps to Check the Ayushman Hospital

1. Open the official Ayushman/PM-JAY hospital search portal.

2. Select your State.

3. Select your District.

4. Choose Government or Private Hospital.

5. Select the relevant speciality, such as Ophthalmology/Eye Care.

6. Enter the hospital name if you have a specific hospital in mind.

7. Complete the Captcha, if shown.

8. Click on “Search”.

9. Check whether the hospital is currently empanelled and has the required eye speciality.

10. You can also check the overall empanelled list of hospital of your district

• CAPF Ayushman Card

• Aadhaar Card or valid ID

• Doctor’s prescription/referral
