6. Enter the exact hospital name.

7. Complete the Captcha, if shown.

8. Click on Search.

9. Check that the hospital has active PM-JAY empanelment for the required cardiac treatment.

10. Visit or contact the hospital's Ayushman Mitra/PM-JAY desk with your RHD diagnosis and cardiologist's treatment advice. Ask them to confirm the exact valve replacement package, type of valve covered, implant/package rules and whether pre-authorization is required.
