Steps to Check Ayushman Hospital

1. Open the official Ayushman Bharat PM-JAY Hospital Search.

2. Select Delhi as the State.

3. Select the relevant District.

4. Choose Government or Private Hospital.

5. Select the required cancer-related specialty.

6. Enter the hospital name if you have a particular hospital in mind.

7. Complete the captcha, if shown.

8. Click Search.

9. Check whether the hospital has active PM-JAY empanelment and the required cancer package.

10. You can also check overall empanelled hospital list of your district.

11. Before starting treatment, visit the hospital's Ayushman Mitra/Ayushman desk and confirm the exact treatment, package, cashless process and pre-authorisation requirements.

Documents to Keep Ready

• Ayushman Card/e-card

• Aadhaar Card or valid government ID

• Cancer biopsy/histopathology report

• Doctor's prescription and treatment plan

• CT/MRI/PET-CT or other scan reports, if available

• Blood test and other investigation reports

• Previous chemotherapy/radiotherapy records, if applicable

• Previous discharge summary and medical records

• Current medicines/prescriptions

• Any documents requested by the hospital for PM-JAY approval
