Steps to Check an Ayushman Hospital

1. Visit the official PM-JAY hospital search portal.

2. Select the State.

3. Select the District.

4. Choose Government or Private Hospital.

5. Select the relevant Specialty as Obstetrics and Gynaecology.

6. Enter or select the Hospital Name if you have any in mind.

7. Enter the Captcha, if shown.

8. Click Search.

9. Check whether the hospital has active PM-JAY empanelment.

Documents to Keep Ready

• Ayushman Card

• Aadhaar Card

• Pregnancy/medical records

• Doctor’s prescription or consultation papers

• Test and scan reports

• Previous treatment records, if any
