Can a urinary tract infection cause high bilirubin levels in adults?

A simple urinary tract infection (UTI) usually does not directly increase bilirubin in adults. However, a severe or complicated UTI, especially one that spreads into the bloodstream and causes sepsis, can sometimes lead to elevated bilirubin.

The important point is to determine whether the bilirubin was high in a blood test and whether the increase is in direct or indirect bilirubin.

How Can a Severe UTI Affect Bilirubin?

A severe infection can affect the body's normal liver and bile-processing functions.

• Severe UTI or kidney infection can cause significant inflammation.

• If the infection progresses to sepsis, liver function can be affected.

• Sepsis may cause reduced bile flow, resulting in increased bilirubin.

• A routine UTI without complications generally does not explain a high bilirubin level.

What Are the Common Causes of High Bilirubin?

High bilirubin can occur because of problems involving the liver, bile ducts or breakdown of red blood cells.

Common causes include :

• Liver infections such as hepatitis

• Fatty liver or other liver conditions

• Liver injury caused by certain medicines

• Gallstones or blockage of the bile ducts

• Excessive breakdown of red blood cells

• Certain inherited conditions affecting bilirubin processing

Your doctor may check total bilirubin, direct bilirubin, indirect bilirubin, ALT, AST, ALP and GGT, along with other tests depending on your symptoms.

UTI Symptoms That Need Medical Attention

A UTI commonly causes burning while urinating, frequent urination, urgency and lower abdominal discomfort.

You should seek prompt medical care if you also develop

• High fever or chills

• Pain in the back or side

• Vomiting

• Severe weakness or confusion

• Very low blood pressure or dizziness

• Yellowing of the eyes or skin

These symptoms can indicate a more serious infection or another condition requiring urgent evaluation.

What Should You Do Now?

Do not take antibiotics or liver medicines on your own based only on the bilirubin result. The UTI and high bilirubin should be evaluated together, particularly when both abnormalities appeared in the same period.

The doctor may review your urine test, urine culture, CBC, kidney function, liver function tests and bilirubin levels to identify the actual cause.
