How to check a PM-JAY hospital in Hyderabad

1. Open the official Ayushman Bharat PM-JAY hospital search.

2. Select Telangana as the State.

3. Select Hyderabad as the District.

4. Select Government or Private hospital.

5. Select the required speciality for your treatment.

6. Enter the hospital name if you already have a hospital in mind.

7. Enter the Captcha, if displayed.

8. Click on Search.

9. Check whether the hospital has an active PM-JAY empanelment and the required speciality.

Documents to carry

• Ayushman Card/e-card

• Aadhaar Card or another valid government ID

• Doctor's prescription or referral

• Recent medical reports

• Blood and urine test reports, if applicable

• X-ray, ultrasound, CT or MRI reports, if available

• Previous discharge summary or operation records, if applicable

• Current medicines/prescriptions
