6. Enter the hospital name, if you want to check a specific hospital.

7. Complete the CAPTCHA, if shown.

8. Click Search.

9. Check whether the hospital is listed as an active PM-JAY empanelled hospital.

10. You can also check overall empanelled list of hospitals of your district.

• Aadhaar card or accepted ID proof

• Mobile number for verification

• Doctor's prescription
