Ayushman Hospital Kaise Check Karein?

1. PM-JAY ki official hospital search website kholen.

2. Apna State select karein.

3. District select karein.

4. Government ya Private Hospital select karein.

5. Specialty mein General Medicine ya Respiratory-related specialty check karein.

6. Hospital ka naam select karein, agar aapko koi specific hospital pata hai.

7. Captcha dikhe to complete karein.

8. Search par click karein.

9. Hospital ka active PM-JAY empanelment check karein.

10. Aap apni district ke overall empanelled hospital ki list bhi check kar sakte ho

11. Hospital ke Ayushman Mitra se confirm karein ki aapki exact saans ki problem aur required treatment ka package available hai aur approval ki zarurat hai ya nahi.

• Ayushman Card

• Aadhaar Card ya valid ID proof

• Mobile number

• Doctor ki prescription

• Purane treatment ke reports

• Chest X-ray ya CT scan, agar hua ho

• Blood test reports

• Pichle hospital ke discharge papers

• Chal rahi medicines ki list

Agar saans bahut zyada phool rahi hai, oxygen bahut kam aa rahi hai, hoth/chehra neela pad raha hai, chest pain hai, confusion ho raha hai ya patient behosh ho raha hai, to Ayushman approval ka wait na karein aur turant emergency department mein le jaayein.
