For eligible ECHS beneficiaries, ECHS provides extensive medical coverage, including outpatient and inpatient treatment, investigations, medicines and treatment at ECHS-empanelled hospitals. There is also no overall monetary ceiling on treatment under ECHS.


However, it does not mean that every service at every hospital is automatically covered at 100%. Coverage depends on the treatment, ECHS rules, hospital empanelment, applicable CGHS/ECHS package rates, ward entitlement and whether the procedure requires prior approval. 


What happens at an ECHS-empanelled hospital?


For covered treatment at an ECHS-empanelled hospital, treatment is generally cashless and the hospital should provide the treatment according to the applicable ECHS package/rates. ECHS specifically states that empanelled hospitals cannot charge beneficiaries for covered treatment. 


The current revised ECHS rates, effective from 15 December 2025, apply to healthcare services at ECHS-empanelled healthcare organisations.


Does the patient have to pay anything?


Usually, there should not be an out-of-pocket payment for treatment that is covered under the applicable ECHS package at an empanelled hospital.


However, expenses can arise in situations such as


  • A treatment or item is not covered under the applicable package.

  • The patient chooses something outside the ECHS entitlement.

  • A non-empanelled hospital is used without applicable ECHS authorisation.

  • An unlisted procedure requires separate approval.

  • A higher/non-entitled room or facility is chosen.

  • An item is specifically excluded from the applicable package.


ECHS package rates are linked to ward entitlement. Under the current rates, the prescribed package is based on semi-private ward entitlement, with adjustments for general/private ward entitlement.


What about treatment at a non-empanelled hospital?


Emergency treatment at a non-empanelled private hospital may be considered for reimbursement under ECHS rules, but reimbursement is generally restricted to applicable CGHS rates or actual expenditure, whichever is lower. 


Therefore, it is better to use an ECHS-empanelled hospital whenever possible and follow the required referral/authorisation process.


How to check whether a particular treatment is fully covered?


Before admission, you should


  1. Check whether the hospital is currently ECHS-empanelled.

  2. Confirm that the required speciality is available.

  3. Ask the ECHS Polyclinic about the referral requirement.

  4. Ask the hospital's ECHS desk whether the exact procedure is listed.

  5. Confirm the applicable ECHS package/rate.

  6. Ask whether any prior approval is required.

  7. Confirm whether implants, medicines, investigations and consumables are included or separately payable.

  8. Confirm your ward entitlement before admission.


ECHS maintains an official list of empanelled hospitals and publishes updated policies and rates on its website.



ECHS provides comprehensive coverage, but not every service is automatically 100% covered without conditions. For a covered procedure at an empanelled hospital, treatment is generally cashless according to ECHS-approved packages and rules.
