What Treatment Is Available for Varicose Veins With Leg Swelling, Pain, and Darkening Around the Ankle?

Visible or enlarged veins around the ankle, leg swelling, pain and darkening of the skin can occur when the veins are not effectively carrying blood back toward the heart. This is commonly seen with chronic venous insufficiency and varicose veins.

The dark skin around the ankle can develop because blood and fluid remain pooled in the lower part of the leg for a long time. If left untreated, some people can develop skin irritation or ulcers around the ankle.

However, a sudden increase in swelling or pain should not automatically be assumed to be varicose veins. A blood clot in a deep vein can also cause leg swelling and pain and needs urgent medical attention.

Which Doctor Should You Consult?

A Vascular Surgeon or a doctor specialising in vascular/vein problems should evaluate the leg.

The doctor will examine the veins and recommend a venous Doppler ultrasound. This test helps check blood flow and whether the valves in the veins are working properly. It can also help rule out a deep-vein blood clot when clinically suspected.

What Treatment Is Needed?

Treatment depends on the severity and the Doppler findings. It can include:

• Compression stockings when medically appropriate
• Regular walking and leg movement
• Keeping the leg elevated while resting
• Medicines for pain or swelling when prescribed
• Treatment of the affected veins, such as sclerotherapy, endovenous laser or radiofrequency treatment, or other procedures when required
• Treatment of any skin changes or ulcers if present

A doctor should decide whether compression stockings are suitable, particularly if there are problems with the arteries supplying the leg.

What Can You Do at Home?

Avoid sitting or standing continuously for long periods. If sitting for a long time, move your ankles and walk for a few minutes regularly.

When resting, keep the affected leg elevated above the level of the heart for some time, if comfortable.

Maintain a healthy weight, stay adequately hydrated and avoid tight clothing around the waist or legs. Do not massage the painful or swollen leg forcefully until a doctor has ruled out a blood clot.

Which Exercises Are Usually Helpful?

Gentle exercises can improve circulation, such as:

• Walking at a comfortable pace
• Ankle pumps
• Moving the ankles in circles
• Gentle calf raises

Start gradually and stop if exercise causes significant pain or increases swelling. Avoid strenuous leg exercises until the cause of the swelling and pain has been assessed.

When Should You Seek Urgent Medical Care?

Get urgent medical attention if the swelling or pain suddenly becomes much worse, especially if one leg becomes noticeably more swollen, warm or red than the other.

Also seek emergency care immediately if leg swelling occurs together with sudden chest pain, breathlessness, coughing up blood, or fainting this can mean part of a blood clot has travelled to the lungs (a pulmonary embolism), which is a genuine medical emergency.

Because you already have pain, swelling and skin darkening around the ankle, it would be better to arrange a vascular evaluation rather than relying only on home remedies.
