7. Complete the captcha, if shown.

8. Click on Search.

9. Check whether the hospital has active PM-JAY empanelment.

10. You can also check overall empanelled list of hospitals of your district.

11. Before admission, contact the hospital's Ayushman Mitra/Ayushman desk and confirm that your specific treatment package is available under PM-JAY.

Documents to Carry

• Ayushman card

• Aadhaar card or other valid ID

• Doctor's prescription

• Medical reports and test results

• X-ray, ultrasound, CT or MRI reports, if applicable

• Previous prescriptions and discharge papers

• Current medicines list

• Any other documents requested by the hospital
