How to Check a Hospital Under PM-JAY

1. Visit the official PM-JAY Hospital Search portal.
2. Select your State.
3. Select your District.
4. Choose Government or Private Hospital.
5. Select the required specialty:
   • Urology, for kidney stones, prostate, urinary problems, etc.
   • Nephrology, for kidney-related medical conditions and dialysis.
6. Enter the hospital name if you already have one in mind.
7. Complete the Captcha, if shown.
8. Click Search.
9. Check whether the hospital is currently empanelled under PM-JAY and whether the required specialty is listed.
10. You can also check the proper list of hospitals which are empanelled.

Documents Usually Required

• Ayushman Bharat PM-JAY Card
• Aadhaar Card or other accepted identity proof
• Doctor's prescription or referral
