2. Select your State.

3. Select your District.

4. Choose Government or Private Hospital.

5. Select the relevant specialty, such as Cardiology/Cardio-Thoracic and Vascular Surgery, if available.

6. Enter the exact hospital name.

7. Complete the captcha, if shown.

8. Click Search.

9. Check that the hospital's PM-JAY empanelment is active and that the required cardiac surgery package is available.

10. Before admission, contact the hospital's Ayushman Mitra/PM-JAY desk with the patient's heart reports and doctor's advice. Ask them to confirm that the required CABG/bypass package is covered and whether pre-authorization is required.
