7. Enter the captcha, if shown.

8. Click Search.

9. Check whether the hospital is PM-JAY empanelled and whether the required cardiac speciality is listed.

11. Before admission, contact the hospital's Ayushman Mitra/Ayushman desk and specifically confirm CRT-D implantation, package availability, device coverage and pre-authorisation requirements.
