How to check the Ayushman Hospital?

1. Go to the PM-JAY Hospital Search.

2. Select your State.

3. Select your District.

4. Select the Hospital Type like public or private.

5. Select the relevant Speciality, such as Cardiology or CTVS.

6. If you know the hospital name, you can enter it.

7. Fill in the Captcha.

8. Click Search.

9. Check the empanelled hospital list.

Documents required

• Ayushman Card/e-card

• Aadhaar Card or valid ID

• Doctor's prescription/referral

• ECG and Echocardiography reports

• Angiography report/CD, if available

• Blood-test reports

• Previous medical records and discharge summaries

• Current medicines/prescriptions
