How to Check a Hospital Under Ayushman Bharat

Follow these steps to check whether a hospital provides pacemaker treatment under PM-JAY :

1. Open the official Ayushman Bharat PM-JAY Hospital Search portal.
2. Select your State.
3. Select your District.
4. Select Government or Private Hospital, as applicable.
5. Select the Cardiology specialty.
6. Search for the hospital by entering its exact name, if known.
7. Complete the Captcha, if displayed.
8. Click Search.
9. Check whether the hospital has active PM-JAY empanelment and Cardiology services.
10. Check proper whole list also which are empanelled under ayushman bharat.

Documents Required

Carry the following documents when visiting the hospital :

• Ayushman Bharat PM-JAY Card
• Aadhaar Card or another accepted identity document
• Cardiologist's prescription or referral
• ECG reports
• Echocardiogram (Echo) report, if available
• Holter monitoring report, if advised
• Previous pacemaker records, if this is a replacement
• Previous hospital discharge summary
• Existing pacemaker/device details, if available
• Other relevant medical reports
• Current medicines and prescriptions
