The G-Card is issued under the AB-PMJAY-MAA framework for eligible Gujarat government employees, pensioners and their eligible family members. The scheme provides cashless treatment for covered procedures at empanelled hospitals.
Go to a government hospital or an empanelled private hospital that has a Cardiology department.
Visit the hospital's Ayushman Mitra/PM-JAY-MAA help desk.
Show your G-Card and Aadhaar or another valid ID for verification.
Take your cardiologist's prescription and all previous heart reports.
The hospital will check your eligibility and the applicable cardiac package.
If required, the hospital will submit a pre-authorisation request to the State Health Agency.
After the required approval, the hospital can proceed with the covered treatment under the cashless facility.

ECG and 2D Echo reports, if available
Blood test reports
TMT, CT coronary angiography or other heart reports, if already done
Previous prescriptions and medical records
