Description
Can Diabetic Nerve Damage Be Repaired?
Dear Ujjwal Jangra ji,
Diabetic nerve damage, also called diabetic neuropathy, usually cannot be completely reversed once significant nerve damage has occurred. However, proper treatment can control symptoms, slow further nerve damage, and sometimes improve nerve function, especially when the condition is identified early.
The main goal is to control diabetes, manage nerve-related symptoms, protect the feet and prevent complications.
Persistently high blood sugar can damage the nerves and the small blood vessels that supply them. Over time, this can affect nerves in the feet, legs, hands and other parts of the body.
The risk can increase with
Poorly controlled blood sugar
Long-standing diabetes
High blood pressure
High cholesterol
Smoking
Excess weight
Vitamin B12 deficiency
Kidney problems
Symptoms depend on which nerves are affected. Common symptoms include
Burning or tingling in the feet or hands
Numbness or reduced sensation
Sharp, shooting or electric-like pain
Increased sensitivity to touch
Muscle weakness
Loss of balance
Feeling as if you are walking on cotton
Reduced ability to feel heat, cold or injuries
Some people may also develop digestive, bladder, sexual or sweating-related problems when autonomic nerves are affected.
A doctor evaluate your symptoms and perform a neurological and foot examination. Depending on your condition, tests include
Blood sugar and HbA1c
Vitamin B12 levels
Kidney function tests
Thyroid tests when required
Foot sensation testing
Nerve conduction studies or EMG in selected cases
The doctor also check for other causes of neuropathy because not every nerve problem in a person with diabetes is caused by diabetes.
Good blood sugar control is the most important step to prevent further nerve damage. Your doctor will set an individual blood sugar and HbA1c target based on your age, health and other medical conditions.
Do not change insulin or diabetes medicines on your own.
If burning, tingling or nerve pain is significant, doctors may prescribe medicines such as
Pregabalin
Gabapentin
Duloxetine
Amitriptyline or other suitable medicines
These medicines require medical supervision because they can cause side effects such as sleepiness, dizziness or swelling in some people.
Vitamin B12 deficiency can also cause nerve problems and may be more common in some people taking certain diabetes medicines. If your B12 level is low, your doctor may recommend supplements or injections.
Do not take high-dose supplements without testing or medical advice.
Regular, suitable physical activity can help improve blood sugar control, strength and balance. Physiotherapy may be useful if neuropathy has caused weakness or difficulty walking.
Exercise should be chosen according to your foot sensation, balance and overall health.
Loss of sensation can make a small cut or blister difficult to notice. Proper foot care is therefore extremely important.
Check your feet every day for cuts, blisters, redness or swelling.
Avoid walking barefoot.
Wear comfortable, properly fitting footwear.
Do not use very hot water or heating pads on numb feet.
Keep the skin clean and moisturised, but avoid applying moisturiser between the toes.
Have foot wounds examined promptly.
Avoid smoking because it can reduce blood circulation.
Keep diabetes, blood pressure and cholesterol under control.
Surgery is not the routine treatment for diabetic neuropathy. Most patients are treated with blood sugar control, medicines, lifestyle measures and management of the underlying cause.
In selected patients, if a specific nerve is compressed or trapped, a specialist may consider nerve decompression or another procedure. Such treatment is different from routine diabetic neuropathy treatment and is not suitable for everyone.
Advanced pain treatments may also be considered when severe nerve pain continues despite appropriate medicines.
Seek medical attention promptly if you develop
A foot wound that is not healing
Black or discoloured skin
Increasing redness or swelling
Pus or foul-smelling discharge
Fever with a foot wound
Sudden severe weakness
Rapidly worsening numbness
Difficulty walking or maintaining balance
A diabetic foot infection can become serious if treatment is delayed.
